I declare that my child is physically fit and ready to participate in Hop Along Yogi kids yoga classes taught by certified Hop Along Yogi instructors. It is expressly agreed that Hop Along Yogi and its agents or employees shall not be liable for claims, demands, injuries, damages, actions or causes of action, whatsoever, to my child or property arising out of or connected with the services provided and of the premises where the same is located.
