Thank you for your interest in the Senior Farmers' Market Nutrition Program.

To qualify for the program, you must:

1. Live in Butler, Clermont, Clinton, Hamilton or Warren counties in Ohio.
2. Be age 60 or older.
3. Have an annual household income that meets one of the following guidelines:

-1 person in household, annual income of $0-$23,107
-2 people in household, annual income of $0-$31,284
-3 people in household, annual income of $0-$39,461
-4 people in household, annual income of $0-$47,683
-5 people in household, annual income of $0-$55,815
-6 people in household, annual income of $0-$63,992

Please continue and fill out the application if you believe you meet the above program qualifications. Note that each eligible applicant in a household must complete a separate application.
