Furry ID Commission Form
Hello! :3 Thank you for your interest in getting a Furry ID! This ID is a fun badge that's meant to look like a driver's license, passport, or similar document, except it is of your fursona or character. It includes details like your name, species, fur and hair color, etc., as well as a drawn "ID photo" of your fursona.

About the Furry ID
A Furry ID is a badge commission which is meant to look like a driver's license, office ID badge, or similar document. It has a lot of information about your fursona or character, like name, species, fur color, etc. The ID commission also includes hand-drawn art of your fursona/character, which is a front-facing headshot, from the shoulders up, facing forward as if looking at a camera, with an expression of your choosing. It'll be used as your "ID photo."

A Furry ID costs $10 USD. The ID will be done in 2 styles, a vertical-style badge, like an office ID, and a horizontal-style badge, like a driver's license. The "ID photo" will be included as a separate file along with your IDs. You'll get 3 digital files in total: both versions of the finished ID, as well as the drawn headshot "photo" used in the ID. Fill out and submit this form to get one!

Please refer to the diagram below to see an example of a Furry ID, and what kind of information will be displayed on it.

The specs of the images are as follows:
> Furry ID (vertical): 1920x3200 pixels, PNG format
> Furry ID (horizontal): 2880x1920 pixels, PNG format
> Headshot: 2160x2895 pixels, PNG format

PHYSICAL BADGE OFFER!

A physical badge is available for purchase for an additional cost. The horizontal (wallet-size) version is $5 USD, and the vertical (badge) version is $10 USD. Shipping is always $5 USD. Both physical versions feature a backside which can include 2 more QR code links, if you desire. It will be shipped in the mail sometime after the digital version is sent to you.

Please link me to references of your fursona/character which I'll use to make your headshot. Close-ups of your character's head would work best, but any references will do just fine. If you do not have image references, write "NONE" and I'll contact you about giving me a description.
NAME *
The name of your fursona/character.
SPECIES *
The species of your fursona/character.
BODY COLOR *
The names you call your body colors. Max 3; List as main color first, then secondary, then tertiary, if applicable.
HAIR COLOR
Color of the hair or whatever serves the same purpose. Max 3; List as main color first, then secondary, then tertiary, if applicable. If there's no hair, write "N/A".
EYE COLOR
If there are two eye colors, list your character's Right eye color first, then the Left eye.
GENDER
If you would prefer not to say, it will be displayed as "WOULD PREFER NOT TO SAY" across Gender and Pronouns. If you have any questions/concerns, please message me and I'll work with you as best as I can.
PRONOUNS
If you would prefer not to say, it will be displayed as "WOULD PREFER NOT TO SAY" across Gender and Pronouns. If you have any questions/concerns, please message me and I'll work with you as best as I can.
HEIGHT
This can be of your character or your own, whichever your prefer. If you would prefer not to say, it can be removed entirely by answering "REMOVE".

YEAR OF BIRTH
This is optional. This, along with Season of Birth, will be displayed in the small text box above the barcode. If you do not want to display these, answer "REMOVE". There is no month or day asked.
SEASON OF BIRTH
This is optional. This, along with Year of Birth, will be displayed in the small text box above the barcode. If you answered "REMOVE" in the box above, this will not be displayed.
COUNTRY
This will display what country you are from, and be shown as a silhouette at the top/center of the ID. If you would prefer not to say, answer "OTHER" and below, you can select a globe showing the continent you are from, or select a world map. If no response is given, the world map will be selected by default.

SIGNATURE
You can send in a custom signature of your character to be placed on the ID below all of your info. You can send it along with your references, or when I contact you later on. A PNG image with a transparent background is preferred. The bigger the size of the image, the better. A signature with a width that is much greater than the height would work best, however, whatever dimensions it is will be made to fit. If you can't provide a digital file, a photo of a signature on paper would work fine.

QR CODE LINK
Provide a link to be displayed as a QR code on the bottom-left of the ID. It can be your social media account, your website, your content page, etc., and I will make a QR code to use.
ID BACKGROUND COLOR
For the background of the ID, not the headshot. There are 12 pre-set colors to choose from, as well as 12 pre-made Pride flag backgrounds available. If you would like a Pride flag not on this list, select "Other Pride Flag" and fill in the next box with the flag you'd like. Check the image below to see what the colors and flags look like. HEADSHOT BASIC INFO
As part of this commission, I will draw a headshot of your fursona/character to be used as the "photo" in the ID. It's a front-facing shot, from the shoulders up. Your character will always face forward toward the "camera," but you can have any expression you'd like, whether it's happy, neutral, angry, sad, silly, annoyed, etc., though you always face forward. When I contact you, we can discuss the headshot in more detail.
HEADSHOT BACKGROUND COLOR
The background for the headshot can be any color, or combination of colors in a gradient (from top/left to bottom/right). Write what color(s) you would like for this background here. You can be as specific as you'd like, with hex codes, RGB values, etc., or a link to a specific color scheme, flag, etc. that you'd like to use.
PHYSICAL ID BADGE

TERMS OF SERVICE:

You are free to use the headshot and the ID files online in any non-commercial manner you would like, including posting it to your social media, making it your icon/profile picture, using it as a reference, etc., as long as you credit me as the artist by using my Twitter handle if posted publically. You are free to print your ID badges or headshot for your own personal use. You are not free to use the files for commercial purposes.

You retain the right to your character and info. By commissioning a Furry ID, you grant me the right to use your Furry ID for advertising purposes, such as using it as an example or some form of display.

No lineart (or coloring) will be worked on until you approve the sketch AND send payment through the invoice. Once the headshot is finished, I will not work on it any further or make adjustments, unless I made a drastic error like coloring something incorrectly, forgetting an important feature/detail, etc., but I won't make minor adjustments.

This is a SAFE FOR WORK type of commission. I will not draw anything that I consider to be NSFW or innapropriate, and you may not use this commission in a lewd manner. You are not allowed to use any part of this commission for non-fungible tokens (NFTs) or anything associated with NFTs or cryptocurrency.

PAYMENT: All payments are made through PayPal. Payments are in USD. An invoice will be sent to you once the sketch for the headshot is approved by you. The invoice will be for the full order of digital IDs, and physical IDs if you order any. There will not be a separate invoice for only the physical items. Money sent to me outside of an invoice will be refunded. However, I will accept tips through the invoice itself, if you would like to use that.

REFUNDS: If the commission has not been completed, you are permitted a partial refund, based on how far along the commission is. For physical badge orders, if they have not been printed, you are permitted the full refund; if already printed, a partial refund; if it has been shipped already, there is no refund available.

NOTE: This is a work of fiction. "Furry ID" badges are only a prop item, and do not serve as legal documents of any kind. Names, characters, places, organizations, and governmental bodies are either the products of the author’s or the commissioner's imagination, or used in a fictitious manner. Any resemblance to actual persons, living or dead, or actual places is purely coincidental.
