Survey on Subjective Quality of Life Measures during the COVID-19 Pandemic
This survey contains a series of questions on various topics related to social connection/isolation, psychological well-being, health, and personal beliefs. The majority of these questions have been adapted for use from previous surveys on quality of life and social connectedness /social isolation research by the UK Government Statistical Service, the Oxford Poverty and Human Development Institute, and the WHO. A few additional questions have been designed specifically to address the COVID-19 pandemic.

As the general public adapts to the social distancing, and isolation measures that have been implemented to reduce the spread of the COVID-19 virus, a unique opportunity presents itself to analyse the impact of these changes.
