Kernsville Read-A-Thon Reading Log and Pledge Total
Please use this form to record your TOTAL minutes read, and your TOTAL pledges raised.

PRIZES:
• ALL participants will receive a certificate and bookmark for participating in Read-a-Thon!
• “Top Honors” medals will be given to students who BOTH fundraise AND reach the following reading goals: Grades K-2: 250 minutes over the three-week time frame, Grades 3-5: 500 minutes over the three-week time frame.
• In addition, there are two “Top Prizes” which are the “Top Reader” and the “Top Fundraiser”. Each top prize winner will receive a $25 Spring Book Fair voucher! Only students who fundraise are eligible for the “Top Prizes”.
• The class in EACH grade level that reads the most minutes will receive a class reward.

DUE DATES: Total Minutes Read, Pledge Total and Pledge Payment need to be submitted by the due date of Friday, March 5 in order to be eligible for “Top Honors” medals and “Top Prizes”.

 *Due to the pandemic, we cannot accept cash donations.  We suggest collecting donations, then writing one check made payable to KERNSVILLE PTO.  Your check may be returned to school in an envelope marked "Read-A-Thon" along with the student's name, grade and teacher, or mailed to Kernsville PTO, 5051 Kernsville Rd., Orefield, PA 18069. *If a family has more than one student participating, a single check or PayPal payment for the total of all students' pledges is encouraged. Each student should still submit a separate Google Form for minutes read and pledge $ collected.
