Returning students entering Grade 6 must complete a 200 word handwritten essay:  “The most important things I have learned at  UC T.E.A.M.S. are _____________________.” Must be emailed to erichardson@ucteams.org.

THIS RE-ENROLLMENT FORM MUST BE RECEIVED NO LATER THAN FRIDAY, FEBRUARY 26, 2024 IN ORDER TO RECEIVE APPROPRIATE PLACEMENT FOR THE YEAR.

**Siblings who are not currently enrolled at  UC TEAMS must submit a NEW APPLICATION (link below) for their appropriate grade with required documents:

- Proof of residence
- Copy of most recent report card
- Birth certificate
- Immunization records
- Handwritten essay if applicable

The sibling policy may be revoked if any of these documents are missing at the time of application.
