I would like to attend all three master classes.

Due to the popularity of the Pilobolus master class, we are requesting that fee of $10.00 to be sent to Heinz Poll Summer Dance Festival PRIOR to the class to guarantee a spot in the class. If you choose to wait to pay the $10.00 until the day of the class, you will go on a "back up list". *

Class Fee Payment Acknowledgment *

I am attending the Pilobolus Master Class and acknowledge that there is a fee of $10.00 that must be paid in advance or the day of the Master Classes.

I am attending all three master classes and understand that advanced payment for the Pilobolus dance class will gureantee me a spot in the class while I can pay at the door for GroundWorks Dance Theatre and Verb Ballet's Master Class.

I am attending Verb Ballet's Master Class and/ or GroundWork Dance Theatre's Master Class and understand that payment can be made ahead of time or at the door.
