This form is for Matching Game to buyback MATCH tokens from Huobi exchange users. Please make sure your information is correct and pay attention to the following details when you fill out the form:

1. Make sure to use a non-exchange wallet address when sending MATCH tokens for buyback. If you are sending MATCH directly from an exchange wallet it may result in the loss of your tokens.

2. The wallet address for MATCH buyback: 0x6d6d0Fc2A60c4F7efb4251Ea0C0e8b29F799524b (ERC-20)

3. Application Time Peridod:
Dec 5 7:00 PM (GMT+8) - Dec 12 7:00 PM (GMT+8) 2022

4. Matching Game will distribute all the USDT within 1 week after the buyback program.

Note: Participants must use a trusted wallet address (metamusk wallet or any other third party wallet) to send the MATCH tokens, not the Huobi exchange address. You need to make sure your trusted wallet address can be used in the Ethereum Mainnet and Binance Smart Chain. If your MATCH tokens are still on the Huobi exchange wallet, please transfer them to your non-exchange wallet. Transferring directly from the exchange may cause a loss of your tokens.
