Please complete the following for all MIDDLE SCHOOL and HIGH SCHOOL students in your family who plan to attend Trinity Student Ministries events.   Rest assured that we will keep this information in our confidential data base for the church.

In order to try and make it easier for parents, we are hoping to obtain permissions, emergency contacts, and pertinent medical information for students (so that parents don't have to keep completing a paper form with this information for each off site event.)  Please contact Lorraine Hallinen at lorraine@trinityepc.org for questions (i.e. to find out if you already completed).
