Move On Up is the new transitions project by Purple Patch Arts, which will work to empower and support young people with learning disabilities and autistic people as they transition into adult services. As part of Move On Up, we will be helping young people identify what interests them and help them understand what community activities are available to them. By filling in this form, you will be helping us to give clear, accurate information about your service to young people.
