1) I understand that by registering my child in Awana, I agree to be contacted by volunteers and leaders associated with McLaurin Baptist Church Grande Prairie, Camp Wapiti, and all associated ministries to receive information on additional programs of interest for my child/children.

2) I understand that my child/children will be participating in physical activities and games, and that with all physical activity there is a risk of injury and consent to my child's/children's full participation in these games. I fully accept this risk and hold harmless from any legal liability McLaurin Baptist Church, The Canadian Baptists of Western Canada, Camp Wapiti, Awana, and any persons involved or associated with these groups and ministries.

2) In the event of an emergency that requires medical treatment for the above named child/children, I understand that every effort will be made to contact both parents/guardians and if necessary the emergency contact person at the contact numbers I have provided. I understand that should emergency personnel or medical professionals be required for treatment of any injury or ailment that occurs during Awana club time and/or activities, I assume responsibility for any and all costs connected to the care and treatment of my child/children.

3) I understand that volunteers and organizers cannot administer medications of any kind to my child at any time. I understand that in order for volunteers and organizers to use an epipen on my child who is having an allegric reaction, it must be provided along with clear instructions for its use and administration; and that by providing the epipen and instruction for its use I am giving consent for it's administration.

4) I understand and consent that my child may be photographed and recorded during the course of normal club activities and that my child may appear in promotional materials, online, in print, or otherwise.

5) I understand that Awana is a spiritual training program found on the person and teachings of Jesus and his disicples as described in the Bible. I understand and consent that my child will be memorizing Bible verses, singing Bible songs, learning Bible stories, and be taught a Bible worldview. I understand and consent that my child will be taught about the immeasurable love of God demonstrated through Jesus' life death and resurrection, according to the writings complied in the Bible. I understand and consent that my child will be taught to trust in Jesus for forgiveness of sin, for moral guidance, for resurrection from the dead, and for eternal life..

6) Should I have any concern of any sort regarding Awana club, any of its volunteers or activities, I recognize it is my responsibility to initiate communication and seek resolution by speaking directly with leadership or those whose actions are cause of the concern in a private manner.

7) If I am volunteering as an adult or a Jr. Leader, I will commit to keeping all required record checks and Plan to Protect training up to date. I agree that actions or lifestyle choices that represent a significant departure from the core values of McLaurin Baptist Church and it's affiliated ministries may be cause for my dismissal from my volunteer role.
