Nominations may be submitted to the LOSH President anytime between July 1 and March 15.

This award celebrates a LOSH member who has gone “above and beyond” the call of duty by putting in more effort, devoting more time, and doing more than the volunteer job requires.  It essentially means the same thing as “going the extra mile.”

Additionally, the award recognizes that the recipient has uniquely fulfilled the threefold mission of LOSH:  to unite the women of Sacred Heart of Jesus Catholic Church in spiritual, social and philanthropic activities beneficial to LOSH, the Parish and the community.

Process for this Nomination:

Go to the LOSH Website at www.hsvlosh.com and go to forms.  Select the link for Going the Extra Mile.  Complete the nomination form and submit.

THE COMPLETED AND SUBMITTED FORM WILL BE EMAILED BACK TO THE EMAIL ADDRESS YOU PROVIDE AT THE TOP OF THIS FORM. YOU THEN NEED TO FORWARD THE COMPLETED NOMINATION TO THE LOSH PRESIDENT AT president@hsvlosh.com

Once received, the LOSH President will redact the nominee’s name and will make copies for the “Going the Extra Mile” Selection Committee.  This committee shall be appointed by the LOSH Vice-President and will be comprised of 3 members of the Executive Committee and 4 Guild members-at-large.  A quorum of 5 is needed for selection.   In even numbered years, the 4 members-at-large will be from Guilds 2, 4, 6 and 8.  In odd numbered years, the 4 members-at-large will be from Guilds 1, 3, 5 and 7.  The “Going the Extra Mile” Selection Committee will meet anytime after the March 15 deadline but before May 1.  The award will be given at the May General Meeting.
