Thank you for your interest in becoming a mentee in the Jean Augustine Centre's Look Up! Lift Up! Program. Please note that this registration form is accepting mentees for our 2nd Cohort which will be commencing at the beginning of February. The form will take approximately 30 minutes to complete.

The information below will be used as a part of broad process to determine a comprehensive understanding of the mentee suitable to match with the mentor in the Look Up! Lift Up! Program for Cohort #2. Since the age of the mentee is under 18 years this form should be completed by a parent or guardian. The Look Up! Lift Up! (LULU!) program is a peer-based mentorship program supporting girls and young women who identify as Black, racialized or belonging to other underrepresented communities within the Greater Toronto Area (GTA).

The program matches Mentees (ages 7-17) with Mentors (ages 18-30) for six months from February to August, 2024 (Cohort #2). The goal of the mentor/mentee match is to establish healthy relationships based on a sense of belonging, mutual respect, communication, trust, understanding, and accountability.

Matches will make a commitment to connect once per week (in person or online) for 1-2 hours between Monday to Friday from 3 pm - 7 pm or Saturday from 9 am - 2 pm.
