Performance Absence Request Form

Please fill out this form for EVERY absence from a performance. This form should be filled out at least TWO WEEKS in advance to be eligible for make-up credit. If there is an emergency, this form must be filled out within 24 hours of the missed absence to be eligible for make-up credit. If no form is filled out at any time, make-up work will not be available, and the student will receive a zero for the performance. Please note that by simply filling this form out, you are only ELIGIBLE for make-up credit, but are NOT guaranteed it. Marching Band Students - multiple absences will result in you being placed on probation until missed information is learned and performance ready.
