Family Sign-Up Form

Wunderbolt, a MIT Media Lab project, is seeking to connect families of 8-12 year olds with trained volunteer coaches who can support and help them get the most out of their summer during the pandemic.

There are two parts of this form. The first part is for parents/caregivers, and it should take less than 5 minutes to complete. The second part can be filled out by your child(ren). This form and your participation is voluntary.

Please note that we have limited spots for session 2. Sign-up is first come first serve and you may be put on a wait list.
