GFWC Tampa Woman's Club Easy Reporting Form
You can use this form to keep track of activities, hours, donated dollars, in kind donations, and any other activity you did for the club.

Complete a separate form for each activity. For example, if you helped plan a fundraiser and you donated items for the same fundraiser you can use one form. If you donated money to a charity and purchased items for a separate fundraising event they would be submitted separately.

Date *
Please enter the date of the activity. If you are bulk reporting enter the date of the event or today's date. Overall the exact date is not important in all circumstances.

What project area does your activity fall under? *
Please select the primary CSP or Project Area that your activity falls under to the best of your ability. Some activities have multiple reporting categories and the reporting chair will review and may re-categorize your submission.
Title of Project *
Please type the title of the activity, project, or non-profit.
Your Name *
Please enter the name of the person that is reporting the activity. If you are a chair, put your name. If you are a member put your name. If you are a chair that is reporting for a member please enter the member's name. You can report for multiple people and use one name as the reporter.
Email *
A copy of this completed form will sent to your email for your records.
 Number of Members Participated. *
List the number of members that participated. If you are only reporting for yourself for an activity enter 1. If you are reporting for everyone attending enter the number of people you are reporting for.
Hours Reporting *
List the number of hours that you completed. Round to the nearest hour.
Monetary Donation *
Enter the dollar amount that you donated directly to the club or to a charity on behalf of the club's name. Round to the nearest $. This would be a check or actual financial contribution to an organization. If it is the cost of a physical item please enter the value into In Kind Donations. Enter 0 if there was not a monetary donation.
 In Kind Donation. *
Enter the dollar value of any In Kind Donation made directly to the club or on behalf of the clubs name. Round to the nearest $. This is for physical items, not checks or cash. Use the receipt amount or an estimated value of the donated items. If there was not an in kind donation please enter 0.
Description of Activity *
Briefly describe your activities, what you donated, and/or who you made a donation to. You can also include a description of the charity if it was a donation or volunteer activity. Be as descriptive as possible and include every detail.
