This form may *only* be completed and submitted by postsecondary administrators (typically, the Head of Music of a particular school, or instructors). If you're a student hoping to apply for the scholarship, you must be nominated first. Feel free to ask your instructors to nominate you if you think you're a good candidate.

Before submitting your nominations, please be sure to fully review the MusiCounts website to ensure you are nominating students who are eligible to receive this scholarship. You can find all the necessary information here: https://www.musicounts.ca/programs-overview/scholarships/

Each postsecondary institution in Canada can nominate up to 3 students enrolled in music studies (degree, diploma or certificate) who intend to pursue a career in one of the following career streams: performing artist, music production/recording arts, or music business.

Institutions may nominate one student per stream. However, it is not required for a student to be nominated in each category. For example, if your school only has students interested in pursuing studies in music business, only complete one nomination, and leave the rest of the form blank.
