PROFESSIONAL REFERRAL FORM (ANDOVER)
A Professional Referral Form can only be filled out by a professional on a clients' behalf. 
If this referral is accepted NotJUST A Pet Food Bank can support a client for up to 1 month via weekly appointments. If longer is needed, a second referral will need to be filled out 
This service is only for use if your client's in a financial position that means their pet will go without food or need to be rehomed. This can be due to:
  • Poor mental health
  • Not entitled to claiming benefits
  • Disabilities
  • Domestic Abuse
  • Homelessness
Examples of Professionals that can make a referral:
Citizens Advice Bureau, support workers, vets, advocates, doctors, probation officers, food banks. 

After reviewing the information we will endeavour to get back to you with an outcome within 3 working days. 
If it's an emergency please call the NotJUST A Pet Food Bank branch nearest to you for a same day appointment. 

A lot of the food will be past its best before date, which is a recommendation of quality, not safety. For more information on the difference between best before dates and use by dates please follow this link . This will also be explained to your client upon their arrival.
