This form is intended for returning OTOB students or students wanting to continue from trial lessons to regular term lessons. For those continuing from trial lessons we will hold your same time slot until the date of your last trial lesson. Before filling out the form please follow the link below to update your billing information if you haven't done so. Go into the "My Profile" tab at the top, then "Billing" to enter your new credit card or ACH account. If we set up your login and password, they will both be your e-mail. All forms will be processed in order of receipt for those who have updated their billing information. In the unlikely event we cannot find a time from your availability below we will contact you however we do recommend leaving as much availability as possible. All time slots are first come first serve. For those who are continuing from trial lessons please note that we will hold your time slot until the last trial lesson date.

Online lessons

What is your preferred lesson time (according to the teaching days listed above for your teacher). Please give us as much availability as possible (Ex. Mondays at 4:30pm is best for us but we could also do Mondays anytime between 6:00 - 7:30pm or Thursdays between 5:00 - 7:00pm.) The more availability you give us the more likely it is that we can fit you in! *

I give On the Off Beat Music School, Inc. permission to bill the following September 2021 payment to my billing account on file upon registration, as well as subsequent monthly payments on the 1st of the month from October, 2021 - June, 2022. *

30 minute lessons = $185.15 ($150.15 monthly payment plus $35 yearly registration fee)

45 minute lessons = $260.23 ($225.23 monthly payment plus $35 yearly registration fee)

60 minute lessons = $335.30 ($300.30 monthly payment plus $35 yearly registration fee

Adults only - biweekly 45 minute lessons = $144.73 ($109.73 monthly payment plus $35 yearly registration fee)

Adults only - biweekly 60 minute lessons = $181.30 ($146.30 monthly payment plus $35 yearly registration fee)
