New student registration is $15 and applies yearly in January regardless of initial sign up and payment.

Make-up, Refund, Credit policy *
I acknowledge.
No Make up classes are allowed without medical documentation
No refunds will be given after the start of the session
If the gym closes temporarily, refunds/credits are at gym discretion
Gym Dress Code and Personal Items *

T-shirts, shorts, leotards are permitted. Sports Bra shirts, crop tops or shirts showing mid-riff are not allowed.
No dangling jewelry is allowed (bracelets, necklaces, long earrings)
No chewing gum or food of any kind
Hair must be pulled back and worn in a way as to not interfere with workout
Gym is not responsible for any personal items brought in that may be lost or stole
Photography:  I understand that photos taken by Golden's Gymnastics may be used for the website, facebook or promotional purposes.

Wavier/AcknowledgementAs legal guardian of my designated student(s), I hereby consent to all student(s) participating in the facility's program(s). I recognize that potentially severe injuries can occur in any activities involving height or motion including, but not limited to, tumbling and gymnastics related activities. I understand that it is the express intent of all staff and personnel to provide for the safety and protection of my student(s) and, in consideration for allowing my student(s) to use these facilities, I hereby COVENANT NOT TO SUE and FOREVER RELEASE  this facility, affiliated and partner companies and organizations, property owners and lessors, staff, contractors, subcontractors, teachers, coaches, owners, directors and other members involved on this facility's program(s) from all liability and for any damages and/or injuries suffered by my student(s) during instruction, supervision, and/or control during any and all classes or extra activities.   *
