Glimmer of Hope Foundation is able to provide packets that Social Workers/Child Life Specialists can hand out to families so that families can apply for a doll for their child. These packets include a brochure, business card and a postcard with information on how to apply for a doll. Social Workers/Child Life Specialists are not required to fill out anything regarding the doll application, once the family receives the packet they can complete the form individually.
