Please complete this form to submit your community involvement hours. You are required to complete a minimum of 40 hours for your Ontario Secondary School Diploma (OSSD). Information will be updated biweekly and can be verified in your MyBlueprint account through your official high school plan.

NOTE: Please include the name of a person for the Community Sponsor :   For example, if you did some time keeping for the Basketball team here at JHSS, the community sponsor might be Mr. McIntyre.  Or if you volunteered at the Food Bank, who was the "person in charge" of the volunteers.
