DATA RETENTION

You have a choice. Either you can complete this before every activity you attend and your data will be deleted after 21 days, or you can grant us permission to hold your data for 3 months + 21 days so you don't have to fill this form out repeatedly.

I agree to abide strictly by the guidelines and public health restrictions set by the NHS, the government, PlayFit Sports & Social Ltd and the sport halls, and to not engage in high risk activity.
