Mindful Soul Center for Wellbeing has put in place preventative measures to reduce the spread of COVID-19; however, Mindful Soul Center for Wellbeing cannot guarantee that you or your child(ren) will not become infected with COVID-19. The waiver, release and other representations and covenants set forth herein are given in consideration for Mindful Soul Center for Wellbeing permitting me to be a patron and utilize its facilities. I agree that I am personally responsible for my safety and actions while visiting Mindful Soul Center for Wellbeing. I agree to comply with all of Mindful Soul Center for Wellbeing policies and rules, including but not limited to all Mindful Soul Center for Wellbeing policies, guidelines, signage, and instructions. I agree to indemnify, defend, and hold harmless the Mindful Soul Center for Wellbeing and its employees, contractors, and other clients or participants from and against any and all costs, expenses, damages, claims, lawsuits, judgments, losses, and/or liabilities (including attorney fees) arising either directly or indirectly from or related to any and all claims made by or against any of the Mindful Soul Center for Wellbeing and its employees/contractors and other clients/participants due to bodily injury, death, loss of use, monetary loss, or any other injury from or related to my use of the Mindful Soul Center for Wellbeing’s facilities, tools, equipment, or materials, whether caused by the negligence of the Mindful Soul Center for Wellbeing and its employees/contractors and other clients/participants or otherwise specifically related to COVID-19. *
