Return your completed form to the parish office, place it in the collection at any Mass, or complete this form online no later than October 8th. There are opportunities for both volunteering and donating.  You do not have to fill out a form if you wish to make a donation.

Family members participating (please list ages of those under 18).  If a family member is participating in a different activity from the rest of the family, please have them fill out a separate application form (e.g. high school student going with the youth group, or family members splitting up because of two different times for activities, etc.) The icon         indicates that the activity is suitable for families with younger children. Service Opportunity Selection
The opportunities are described on the PDF form as indicated at the top of this page.  Please indicate your 1st, 2nd and 3rd choices.  You will be notified as to your placement.  

For each site we need a team captain to act as point person for that site.  The team captain will be responsible for contacting all team members before the day and to be in contact with the agency contact person.  The team captain must have a cell phone and be able to use e-mail.  Please indicate if you would be willing to be a team captain by choosing it in the Team Captain section below.  Time commitment to be a team captain is minimal. Willingness to be a team captain usually assures you your first choice placement.

Other Opportunities on that Day
In addition to the work sites, there is also an opportunity to get involved in the day by donating needed materials ahead of time.  Bins will be in the gathering space beginning September 28.

Cell Phones:
Drop off any cell phones you have which you no longer use. The cell phones will be taken to Verizon.  The memory is purged and the phones are used to help survivors of domestic violence.   Drop phones off in the gathering space or parish office before that day.

Other Donations:
Bubbles, coloring books, crayons, suduko puzzles, children’s craft supplies, sealable plastic bags in sandwich size and gallon size. Toiletries (soap, toothpaste, deodorant (men and women’s), toothbrushes (full size), hand lotion; new white socks for adults. Adult backpacks; used luggage on wheels. We are also collecting items for the Animal Welfare Association: small Ziploc bags, medium kitchen garbage bags, towels, washcloths and non-fitted sheets (can be used, but should be clean). There will be baskets in the gathering space for these donations.  We are also collecting women’s handbags filled with toiletries.

Gift cards needed to support Mitzvah Day projects:  
Any major food store (needed for food prep); Lowe’s or Home Depot (for the gardening and home improvement project sites) and A.C. Moore, JoAnne Fabric or Target (for the craft kits for children in waiting rooms and fleece baby blankets). Gift cards and donations can be placed in the collection at Mass, preferably in an envelope with Mitzvah on the front or dropped off at the parish office.  We also welcome gift cards for BJ’s where we can get supplies for any of these projects.
