This form will be reviewed by the principal of the school you are requesting and by Carrie Jarred at District office.  If you have any questions about the process or need help, please contact Mrs. Jarred  at cjarred@pv362.org.  If your request is approved by the building principal it will be presented to the Board of Education at their next scheduled meeting.  Mrs. Jarred will contact you after the board meeting
