This form is to sign up to attend in-person for the Regular School Board meeting at 5:30 p.m. Wednesday, May 19, 2021, in the Brookings-Harbor High School Library.

In-person attendance may be limited due to space capacity limits and COVID-19 restrictions outlined in ODE's Ready Schools, Safe Learners guidelines.

This form will also serve as a contact tracing form for in-person attendees. All in-person attendees must wear a face mask or full-face shield and maintain physical distance from other attendees. The meeting will also be broadcast via the school district's YouTube Live page with video available live during the meeting and saved for later viewing.

If you can not attend in person, or do not wish to, and still want to provide public comment for the meeting, please email your written comment to nathanh@brookings.k12.or.us at least 24 hours in advance of the meeting for inclusion in the public record.
