Application Instructions

Applications must be filled out completely. Please note that applying does not guarantee an interview or a spot in the Pre-Apprenticeship program. The application deadline is August 25, 2023.

All information in this application will be used by Chef Ann Foundation staff to better understand each potential Pre-Apprentice's abilities, situation, and needs. Information will be kept confidential and will not be shared beyond work sites (K-12 districts) and the employer of record (Career Catalyst). I am eligible to work in the United States (an I-9 will be required if accepted into the program)

I understand that a background check will be required (any associated cost will be covered by the program)

I understand that a physical exam and TB test may be required by my host K-12 district

I understand that travel to work site(s) within the K-12 host district will be required

I understand that I will be required to access supplementary instructional materials in a virtual (online) format

I understand that I will be required to obtain a ServSafe Certificate or State/County Food Handler’s Permit by completion of the probationary period (cost will be covered by the program)

I have the ability to lift up to 50 lbs on a regular basis

I have the ability to stand for extended periods of time
