Pulses reflect the health of our body. The purpose of this questionnaire is to assess the pulse of our church ministries to identify where we are in serving the Lord spiritually, mentally, and physically. This is an anonymous questionnaire, which may take 15 - 30 minutes to complete.

We sincerely welcome your thoughtful comments. Please share which areas of the church ministries are doing well and which aspects of the church ministries need to be strengthened!  May the Holy Spirit guide you to answer this questionnaire. Your answers could help brothers and sisters to grow and work together more effectively in the Lord. May we  experience more "how wide and long and high and deep is the love of Christ" (Ephesians 3:19) while we work together in the church ministries.

Please complete this questionnaire by August 27. We will analyze and summarize survey results and then share results in the Co-worker Retreat. We will share results with church ministry teams as well.
