If your company plans to use ANY non-official, exhibitor appointed contractor (EAC) for labor supervision, this form must be completed by a representative of the Exhibiting Company.
An Original Certificate of Insurance must also be submitted by your EAC in order for them to gain access to the Exhibit Hall.
