Hardwick Evangelical Church: Service Sign-up for Sunday 25th October, 2.45-3.30pm
Hardwick Evangelical Church is holding regular services (check www.hechurch.co.uk for dates), but due to the current restrictions in place, it is essential that you book to attend the service. If you come along to a service without booking a place, you may be turned away, as there is limit to the number of people we can have in the building for social / physical distancing.
There will continue to be weekly church services on Zoom / YouTube in addition to these services.

Considering the limited seating, it may not be possible to accommodate everyone who completes this booking form. If this is the case, you may be contacted and guaranteed priority for the next service. Please sign up before midnight on the Wednesday before the service, and you will be contacted by email on Thursday or Friday to confirm your place.

Requirements for attending the service:
Strict social distancing of 2m must be maintained, before, during and after the service.

On arrival you will be asked to confirm the contact details you have given on this form.

Your details will be kept for 21 days, for Test and Trace purposes.

You will be asked to confirm that you do not currently have any of the key symptoms of COVID-19 (a high temperature; a new continuous cough; a loss of, or change to, your sense of smell or taste) and have not had such symptoms for 14 days, or have been asked to self-isolate by NHS Test and Trace, or have not returned from foreign travel requiring you to isolate.

If you later develop COVID-19 symptoms or test positive then you must contact the church as soon as possible and let us know. You can do this by emailing hardwickevangelical@gmail.com

If you cannot come for another reason, it would also be helpful to let us know, so that we can offer a place to someone else.
I confirm that I agree to the above terms for the requirements for attending the service: *

Practicalities of how the service will be run:
There will be a one-way system in place for going through the building, which will be signposted.

A steward will direct you to your seat, and we ask that you remain in this seat for the duration of the service, unless you need to use the toilet, or have been asked to be involved in the service.

Please only use the toilets if absolutely necessary. If you do need to, you will need to clean the toilet after you have used it.

You will be required to sanitise your hands on entry and exit to the building.

You will be required to wear a face-covering throughout the service, unless exempt (see additional question).

Unfortunately there will be no congregational singing, in line with government guidance.

There will be no Bibles or other physical resources available to use during the service, so you will need to bring your own if you would like to. For Communion services, there will be pre-filled individual cups with a wafer available, that the church has bought.

We will be using the plastic chairs, so if you would like to bring a cushion to make it more comfortable, please do so. Please let us know if this will be a problem.

We will be keeping doors open for ventilation, so you may want to dress warmly, particularly as the weather becomes colder.
A steward or service leader will give guidance about leaving the building.

I understand that the service will be run in the ways described above: *

Face Coverings:  Wearing a face-covering throughout the service is mandatory, unless you are exempt. If someone is not wearing a face-covering, a steward may ask the person to put one on.  There is no requirement to let us know if you are exempt, but if you would like to, you can add this information here (either with or without the reason).
These other people from my household / support bubble, will be attending the service with me:
