I, PARENTS/GUARDIANS, the undersigned, being the parent or legal guardian of the child named below, understand and acknowledge the following:

Assumption of Risk – I acknowledge that participation in physical activities, including but not limited to sports, dance, and wellness programs, involves inherent risks, including injury or illness. I voluntarily assume all such risks associated with my child's participation.
Medical Authorization – In the event of a medical emergency, I authorize Anhao Wellness staff to seek necessary medical treatment for my child. I understand that I am responsible for any medical expenses incurred.
Liability Release – I hereby release, waive, and hold harmless Anhao Wellness, its employees, instructors, and affiliated entities from any and all claims, demands, liabilities, or causes of action arising out of my child's participation in the camp, including injury, illness, or loss of personal property.
Code of Conduct & Dismissal – I understand that my child must adhere to all camp policies and behavior expectations. I acknowledge that repeated misconduct may result in dismissal from the program without a refund.
Photo and Video Consent – I consent to the use of my child's photographs or videos for promotional purposes unless I provide written notice opting out.
Cancellation & Refund Policy – I acknowledge the camp’s cancellation and refund policy, including the no-show and make-up class provisions.
