Please complete the following form to register your family for our family-based Faith Formation program GIFT (Generations in Faith Together), Sacramental (First Communion and/or Confirmation) preparation and our Middle School (MSYM) and High School Youth Ministry Programs (HSYM).

Families whose children will be preparing to receive First Holy Communion must participate in GIFT as this is our primary form of faith formation for families. We believe that faith formation is a life-long endeavor, so continuous, non-interrupted participation in GIFT is recommended for families with children in Kindergarten through 8th grades. Young people you are preparing for the sacrament of Confirmation are encouraged to participate in our High School Youth Ministry program (HSYM).
