Youth/Children Ministry Volunteer Form 
Per the new child protection policy, all volunteers must complete the following application. The application will be reviewed by the pertinent youth and children's ministers. All information submitted on this form will be kept confidential and accessible only by the youth and children's ministers in charge of the ministries. 

By submitting this form, I agree to uphold Chinese Church in Christ North Valley (CCICNV) statement of faith and I affirm that the information contained in this application is correct to the best of my knowledge. I give my authorization to CCICNV  or its representatives to release any and all records or information deemed relevant by CCICNV to the position. CCICNV may contact my references and appropriate government agencies as deemed necessary in order to verify my suitability as a youth/children volunteer. I understand that the personal information in this application will be held confidential by the professional church staff.
