TERMS AND CONDITIONS:   By completing and submitting this form, I hereby enroll myself and my family in the Team Triple Threat Program for the year 2023 - 2024 and give my children permission to play and participate in all activities and programs run by the T3 during that period.  For myself and my family I hereby assume all risks and hazards that are incidental to such participation, and further do hereby release, waive, absolve, indemnify and agree to hold harmless Team Triple Threat, its Board of Directors, its officers, its staff, its employees, its volunteers and its coaches from any injury, damage or claim whatsoever that might arise from this participation in T3 programs regardless of the cause.  I acknowledge that this Release and Waiver is given voluntarily with full knowledge and understanding of the substantial rights I am relinquishing for myself and my entire family. *

PAYMENT:
At this time, the cost for tryouts is $20 per child.  Once tryouts have concluded,  a separate notice will be sent with the final cost for the season and uniform.
