Los Angeles Senior/Disability/Chronically Ill/Immune Compromised Mutual Aid Form

In the midst of the COVID-19 pandemic, we are offering our support to physically disabled, senior, chronically ill and immune-compromised Angelenos to ensure they have the resources, support and care that they need.

At Care Collective LA, those of us within these communities have identified how our needs can be best served. We are entirely volunteer-based and lead by organizers from these specifically vulnerable communities. We will be raising funds and connecting volunteers to work to make sure those most at-risk in this pandemic don't go without basic needs.

1) Basic contact information
2) What kinds of assistance you need
3) If there are any supplies or support you are open to donating
