This is for residents of Carey Exempted Village Schools only.

KINDERGARTEN SCREENING WAS HELD ON APRIL 1, 2022 - PLEASE CALL THE OFFICE 419-396-7922 OPT.2

If you live in another district and are considering open enrolling to Carey Schools, please contact Mrs. Lesa Hyatt at 419-396-7922. She will provide you with the required open-enrollment application. You may also download the application from our website. Please note the OE window does not open until May 1st.

All prospective kindergarten students must be FIVE years old by August 1, 2022.

Step #1 - Complete this Google Form

Step #2 - We are asking that all parents please register on Final Forms prior to their scheduled appointment.
To do so please go to www.careyschools.org and click on the Final Forms link and be sure to select the 2022-2023 School Year. With using Final Forms ALL Parents MUST have a valid e-mail address as paper forms are not an option.

Step #3 - You will need to bring with you the following items: birth certificate (not a mother's/hospital certificate) and immunization records. Custody papers if applicable.

Kindergarten Screening will be held on Friday, April 1st at Carey Schools, 2016 Blue Devil Drive.
Sessions will start at 8:30 with groups scheduling every 15 minutes and the last group starting at 10:15 am.

Parking will be available in the main lot (North side). We ask that you enter through Door #1. Screening will be held in the auxiliary gym.
