Use this form to tell us what food you’d like to receive each time you come to the Food Closet.  If there are things that you need occasionally, but not every week, don’t mark them here. You can just ask for these items when you pick up your groceries.

We’ll do our best to give you all the items you ask for, but we can’t guarantee that we’ll always have everything on your list. We’re limited to the food that is available to us and our stock will vary from week to week.

We’re no longer able to accept changes/additions to your list by phone/text/email.
If you want to make a change to your list, please let your server know.
