Porter Ridge High School Students and Parents,
Beginning Monday, May 11th, we will resume completing N.C. Driving Eligibility Certificates for students who have successfully completed the drivers ed course. Due to the COVID-19 pandemic, the school building is closed, however, the following arrangements have been made to provide students with the completed form. If you need a completed N.C. Driving Eligibility Certificate, please complete the form below. You will be contacted by someone from the school within 24 hours of this form being completed. If you complete the form over the weekend, you can expect a response the following business day. A member of our office staff will work with you to arrange a time to meet at the school at a designated day and time to complete the form for you. To ensure your safety and the safety of our staff, these appointments will be done by appointment only and our staff will follow appropriate health and safety guidelines.  

*Please note...in order for our staff to be able to complete an eligibility certificate, the student must have passed at least 3 classes in the previous semester (1st semester). Student/parent must show the staff member the white drivers ed completion form and a copy of the students birth certificate in order to receive the eligibility certificate.
