Fill out this form to save your spot in Ballerino Creamery's Winter Cheese Club, running January to March 2021. Pickup will be the last Friday of each month (Jan 29, Feb 26 or Mar 26, 2021). Choose to pickup from the Creamery in Staunton (765 Middlebrook Ave) or select our Covid-safe home delivery ($8 delivery fee for locations within Staunton City Limits).

Your spot in Winter Cheese Club is not official until we have received full payment ($45 / month x 3 = $135 for pickup option or $159 for home delivery). All payments must be received by December 1, 2021. You can pay online (we will send you an electronic invoice by email) or you can mail a check made out to: Ballerino Creamery to: 765 Middlebrook Ave, Staunton, VA 24401.
