CUPE 1698 Union Hours Claim Form
This form is for members of CUPE 1698 who are booked off from work on union time to submit their hours to the Treasurer. It is meant to record each period of time that you are booked off for a specific reason on a specific date or pay period. Here are some examples of how to code it after you have entered your name and date/pay period information:
Partial Day Book Off
: You took off a few hours to attend an online CUPE education class, so you select
4 hours
for the number of hours booked off and
Education
as the reason.
Full Day Book Off
: You took a full day off to work on our local's finances, so you select
7 hours
for the number of hours booked off and
Executive Business
as the reason.
Multiple Day Book Off
: As part of your contribution to the union movement, you teach and facilitate CUPE courses and were needed to teach at the week long Fall School in Nanaimo. For each day of the week long school, you would select
7 hours
for the number of hours booked off and
CUPE National Business
as the reason. After submitting you would click
Submit another response
to fill out this form for each individual day of the book off and submit it multiple times.
Multiple Partial Days Off
: You are the Treasurer were booked off the entire day to work on the union's finances, and the Bylaws Committee coincidentally scheduled an afternoon meeting as well. You would select
3.5 hours
for the half day you spent working on the books and
Executive Business
as the reason. You would then click
Submit another response
and fill out the form a second time for the other 3.5 hours.
