Pre Fundraiser Form
If your club is interested in hosting a fundraiser, please complete this form no less than 8 business days prior to the scheduled event. The club will receive an email confirming or denying your clubs request no less than 3 business days prior to the event date. 

Should any changes or updates need to be made to your club's request, please communicate this with the Sport Club Office immediately. 

Note: To receive compliance points, club must complete a Post Fundraiser form within two business days after the event has concluded.
