OSDA Equity Concern Reporting Form
Any tournament attendee who believes that they have been harassed or discriminated against in violation of the OSDA's Ethical Standards, or who has observed harassment or discrimination, may complete this form to report an alleged violation. The relevant sections of the OSDA Ethical Standards is below. A full copy of the OSDA's Ethical Standards can be on the OSDA website (OSDA >> Ethical Standards).

Please note: Equity concerns should be brought to the Equity Office if someone feels they have faced or seen discrimination or harassment, while other issues or protests should be brought to the tab room.

After this form is submitted, an equity officer will contact the coach of the individual who submitted the form and conduct an informal discussion about the matter. After the informal discussion, the equity officer shall investigate the allegations in the form. All concerns will be investigated as soon as possible and the coach and individual who submitted the form will be contacted in a timely fashion after submission. At the conclusion of the investigation, the individual submitting the form will be informed of any applicable resolutions, if appropriate.

Equity officers will keep the information disclosed as confidential as possible. Equity officers are required to take appropriate action and notify authorities if students engage in behavior that presents clear and imminent danger to themselves or others, in the case of known or suspected child abuse or neglect, or in the case of known or suspected sexual violence.

In an emergency, tournament participants should call 911.


OSDA Ethical Standards:
In order to promote fairness, integrity, educational growth, and safety, the Ohio Speech and Debate Association expects all individuals who participate in speech and debate activities, including students, coaches, judges, and other attendees to adhere to the standards outlined below.

The OSDA:
- Welcomes everyone, regardless of age, disability, gender, national origin, race, religion, sex, sexual orientation, or political affiliation, to participate with equal opportunity in the Speech and Debate community within our participation guidelines;

- Commits to providing a safe, inclusive, accommodating environment for all members of our community, free from discrimination, harassment, and related misconduct;

- Expects all members of our community—students, coaches, and judges—to conduct themselves professionally and adhere to directives by officials at Speech and Debate tournaments;

- Asks all participants to examine implicit or explicit personal biases and question how those biases may affect participation in OSDA events; and

- Encourages all members of the community to monitor their ethical behavior by upholding these ethical standards.
