By filling out this form you are applying to become a member of the MOD Holders Association under Swiss law.

To become a member of the MOD Holders Association, your application must be approved by the board and the annual membership fee of CHF 10 must be paid per crypto (ETH/DA/USDC). This fee is needed to limit liability to the association. This fee is not used to cover legal costs. We may ask for some extra money for that, if we need to.
