Tiffness Fitness Application Questionnaire 
This is an intake questionnaire to work with Tiffany / tiffness fitness to learn more about your goals, equipment, injuries, and more. Everything is set as "long answer" but answers can be as short or long as needed. There are no right/ wrong answers here, just answer truthfully so I can gather information to make sure I can be the best coach for you and make sure we can work together effectively.

What equipment and how much space do you have for your workouts?  *
Are you looking for live zoom coaching (30 / 60 mins) or online programming or a mix of both or something different? 
How long do you prefer your workouts to be? And how many days per week do you train?  *
Do you have any current injuries? Any past injuries I should be made aware of? Is there anything related to your body that could affect your training? (i.e; heart problems, pregnancy, chronic illness, surgeries)  *
What kind of feedback do you want from me? Meaning, what kind of support or feedback do you want from me with your training and how often are you hoping to be in contact throughout the week? What communication styles work best for you? And in all forms of this, I will still be giving you technical feedback on your movements if applicable, but I am looking for what is the best way that you are going to take that feedback from me.  *
