By completing and submitting this formor participating in this event, I am granting Ohio Power Fastpitch unrestricted permission to use photographs, video recordings, my name and/or my company’s name for publication -includes electronic, broadcast, and physical format - in fundraising and/or public relationsefforts such as newsletters, brochures, newspaper articles, website, displays, videos, etc.This unrestricted agreement includes materials provided by me or taken by or on behalfof Ohio Power Fastpitch. Information collected is for internal and communication use onlyand will not be sold, distributed, or made publicly available ever. 2021 COVID-19 Release Agreement I hereby, for myself and/or my child understand the risks involved and hereby consent for myself and/or my child to participate in activities and/or use the facilities of Ohio Power Fastpitch and Warren County Youth Baseball. I hereby release, indemnify, and hold harmless Ohio Power Fastpitch and Warren County Youth Baseball, its Commissioners, Agents, Employees and Assignees from any and all liability claims, actions, demands and judgments arising out of injury, illness or loss sustained by me or my child while participating in activities or using facilities of Ohio Power Fastpitch and Warren County Youth Baseball and understand to follow ALL State of Ohio and CDC guidelines. I further explicitly understand and acknowledge that COVID-19, a contagious virus that spreads easily from person to person, remains an ongoing health concern. Federal and state authorities recommend social distancing to prevent the spread of the virus. Ohio Power Fastpitch and Warren County Youth Baseball in no way warrants that COVID-19 infection will not occur through participation in programs held by Ohio Power Fastpitch and Warren County Youth Baseball. I understand that Ohio Power Fastpitch and Warren County Youth Baseball will not be liable for an injury or illness sustained by myself or my child. *
