This form is for collecting continuous feedback. Feel free to leave any suggestions or feedback you may have about the club at any point. Topics we want to hear your feedback about include, but is not limited to, how we run club events, our newsletter, our Instagram, and our Discord server. You can still fill out this form if you would like to remain anonymous.
Another purpose of this form is to give a private and anonymous space for reports on individuals who have breached personal boundaries, whether the boundaries were breached in-person or online. In the event someone is reported for breaching personal boundaries, the reported person will be banned from our Discord server or any other social media as needed, as well as uninvited to future events as we investigate and evaluate the situation. Depending on how the situation resolves, they may be banned from the club indefinitely. We take these reports seriously and will always prioritize the safety of our club members first and foremost.
