Epic Hockey Stars indemnity
I wish for my child enrolled on this form to participate in hockey practices and matches (hereinafter referred to as "the sport") during 2019
Therefore I hereby:
Acknowledge that I have familiarised myself with the nature and extent of the Sport and the risks and dangers to which my daughter/son may be exposed as a result of their participation in the Sport:

Voluntarily assume the risks and dangers to which they may be exposed as a result of their participation in the Sport;

Indemnify and keep indemnified Epic Hockey Stars (the Club) the clubs employees and contractors and/or any persons who represent the Club against  all injury, loss, damage, theft, costs and/or expenses which I, my child and/or any other person may sustain or incur as a result of their participation in the Sport and:-

Agree not to hold the Club, the Clubs employees and contractors and/or any persons who represent the Club  liable for any injury, loss, damage, costs and/or expenses which I or my child may sustain or incur as a result of their participation in the Sport.
