Thank you for your willingness to mentor a preservice teacher during their culminating, year-long clinical practice internship (student teaching). Your commitment to grow and develop the next generation of educators is truly invaluable.

For sections 1-3 below, please provide your personal information, as appropriate. Section 4 requires that you watch a video of an authentic lesson and evaluate the lesson using the Observation & Conference Report indicators provided below. While this is a mock observation, we kindly ask that you treat it as a real observation.
