Please complete the information below to receive breakfast and lunch. This will assist us in making this program meet the needs of our students and their families. You will only need to complete this form one time. We will prepare meals for those listed on the form for the three days not in school.  You will no longer need to call in on a daily basis to order meals unless you wish to cancel. You may still call in if needed to order meals.
